Failure to Show Up for ACFT

PURPOSE OF COUNSELING

Discuss the Soldier's failure to show for the Army Physical Fitness Test (For Record/Diagnostic).

Discuss the possible methods to correct substandard performance.

Discuss the possible repercussions of consecutive Record APFT failures.

KEY POINTS OF DISCUSSION

__NAME__, at __FIRST FORMATION TIME__, on __DATE __, you failed to be at your appointed place of duty for a record/diagnostic APFT, at __LOCATION__. Your failure to show for this scheduled event resulted in a failed APFT.

You failed to meet the APFT standard under AR 350-1 by not reporting to the scheduled APFT formation, which is an unexcused absence. All Soldiers must attain a score of at least 60 points on each test event and an overall score of at least 180 points which you failed to accomplish by not being at the __LOCATION__ as scheduled. Soldiers without an excused absence will be retested the following day or no later than 30 days following the initial APFT no show failure. Commanders may allow Soldiers to retake the test sooner if a Soldier and the Commander feels the Soldier is ready. Personnel that repeatedly fail to report for the record APFT will be separated from service in accordance with AR 135-175 and AR 135-178.

You are counseled for the above indicated unsatisfactory duty performance in accordance with AR 135-178, paragraph 2-4 or AR 635-200, paragraph 1-16. Continued unsatisfactory behavior may result in your separation from the Army. If you are separated for entry level performance and conduct, your service may be uncharacterized. If you are separated for unsatisfactory performance, you could receive an Honorable Discharge or possibly a General Discharge. A General Discharge could prejudice you in civilian life and preclude you from reenlistment. Additionally, a General Discharge could deprive you of all educational benefits. Note that a successful application to upgrade an unfavorable discharge is unlikely. Any separation prior to your expiration term of service could result in you having to pay back unearned portions of bonuses. Your type of discharge will become a matter of permanent record, and may, consistent with the Privacy Act, be provided to Federal, state and local agencies.
