Purpose of Counseling
The purpose of this counseling is to document the requirement for all Soldiers in this section to complete their required training and to document your failure to do so.

Key Points of Discussion:
It is each Soldier's responsibility to make sure they complete required training as required and to keep themselves current in all recurring training.
Your required training is listed in your training plan and we are all informed via e-mail of special training requirements.
You were told that SARC training and Suicide Awareness would only be available at the base theater on two different days but did not go to either briefing.
Because of your failure to follow instructions, this unit is the only unit in our Command that has been flagged as not being 100% and has attracted the attention of the Commander. Because of your failure to follow instructions, MSG Smith must coordinate with Command HQ to determine an acceptable alternate method of training.
AUTHORITY: 10 U.S.C. 8013. PURPOSE: To obtain any comments you desire to submit (on a voluntary basis) for consideration concerning this action. ROUTINE USES: Provides you an opportunity to submit comments or documents for consideration. If provided, the comments and documents you submit become a part of the action. DISCLOSURE: Your written acknowledgment of receipt and signature are mandatory. Any other comment or document you provide is voluntary.

OTHER INSTRUCTIONS
This form will be destroyed upon: reassignment (other than rehabilitative transfers), separation at ETS, or upon retirement. For separation requirements and notification of loss of benefits/consequences see local directives and AR 635-200.

Plan of Action:
Check with me daily for instructions. When an alternate method of training is available I will notify you. At that time you will make SARC and Suicide Awareness training your priority and complete it as soon as possible.

As stated above, make SARC and Suicide Awareness training your priority when a method of training becomes available. In the future, comply with all training requirements or your failure will be met with more serious consequences.
