PART II - BACKGROUND INFORMATION

  Purpose of Counseling


  1-May-13. Performance-oriented counseling. Violation of Article 92: Failure to obey order or regulation.

On 30 April 13, SFC Johnson was derelict in the performance of her duties. SFC Johnson was told to consolidate the battalion briefings. You were given instructions on what was required and the deadline by myself and MSG Ginetti. You told me that you had to pick up your son from daycare before 1700 or you would be charged overtime. I told you yesterday that you may have to work late and to make arrangements for today. You did not make arrangements and because I am trying to work with you, I told you to pick up your son and then come back to work. Instead of returning, you called and said that your slides were complete and that, if necessary, you would come in early and make any last minute corrections. I told you to call me back in 15 minutes after I had a chance to verify your statement. You did not call back, you did not come in early and your briefing slides were not complete. SSG Atkins and I worked until 2300 to complete your assignment and due to his familiarity with the project, took over to meet your responsibilities.


PART III - SUMMARY OF COUNSELING
Complete this section during or immediately subsequent to counseling.

  Key Points of Discussion:

SFC Johnson, I am counseling you for the conduct stated above. Completing assignments, following and carrying out orders and regulations is literally an indispensable trait of an NCO. Orders and directions that are given by the leadership of our unit and the Army must be obeyed without question. You have demonstrated that you lack the self-discipline and motivation or willingness to carry out the duties assigned to you. I have lost confidence in your leadership ability and competence which undermines our unit's cohesion and effectiveness.

This disregard for your responsibilities and for my authority is behavior I would not accept from a Private, much less a senior NCO. I am amazed that I have to counsel a Sergeant First Class as if she were a Private. If this insubordination occurs again, more serious corrective action will be taken.


If this conduct continues, action may be initiated to separate you from the Army under AR 635-200. If you are involuntarily separated, you could receive an Under Other Than Honorable Conditions discharge. A discharge Under Other Than Honorable Conditions is based upon a pattern of behavior or one or more acts or omissions that constitute a significant departure from the conduct expected of a soldier. An Under Other Than Honorable Conditions discharge may be awarded for separation under Chapter 14 for misconduct. If you receive an Under Other Than Honorable Conditions discharge, you will be ineligible for reenlistment and for most benefits, including payment for accrued leave, transitional benefits, the GI Bill and transportation of dependents and household goods to home of record. You may also face difficulty in obtaining civilian employment, as employers have a low regard for Under Other Than Honorable Conditions discharges. Although there are agencies to which you may apply to have the character of your discharge changed, it us unlikely that any such application will be successful.


OTHER INSTRUCTIONS
This form will be destroyed upon: reassignment (other than rehabilitative transfers), separation at ETS, or upon retirement. For separation requirements and notification of loss of benefits/consequences see local directives and AR 635-200.

You have been relieved of duties as Operations NCO. SSG Atkins will assume duties as Operations NCO effective today. Report to CSM Hawkins' office for a new assignment/duty position.

SFC Johnson has quickly adapted to her new responsibilities as Barracks NCO and continues to make positive contributions to this organization's mission.
