SPC Snuffy, your leadership has noticed a pattern of lateness in regards to arriving for first formation. You have received verbal and written counselings on the importance of reporting to first formation and the consequences of failing to report. Within the past 60 days you were given a written counseling warning that if you received another counseling for missing first formation, punitive action would be recommended against you under Article 92 of the Uniformed Code of Military Justice.
Regardless of any excuse or reason you may have for tardiness, it is absolutely necessary that you are present for duty and accounted for at all times, and especially for first formation. Your current behavior is unacceptable and will not be tolerated. You are hereby warned of the intent of your leadership to seek UCMJ action against you if you are late to first formation after the receipt of this counseling.

OTHER INSTRUCTIONS
This form will be destroyed upon: reassignment (other than rehabilitative transfers), separation at ETS, or upon retirement. For separation requirements and notification of loss of benefits/consequences see local directives and AR 635-200.

Plan of Action:
We will review your daily routine and identify the cause of your frequent lateness. We will determine how long it takes to drive to work and set a time for you to leave your home that allows for unexpected traffic problems. We will work backwards from that time to determine an appropriate bed time. We will test your alarm clock and look for any other obstacles to showing common courtesy to your unit.

As your supervisor, it is my responsibility to ensure you are treated fairly. It is also my responsibility to meet my obligations as an NCO to the rest of the platoon and to the unit and enforce standards. If you do not report for duty on time one more time, I will recommend you be flagged/barred from reenlistment.

SPC has not been late for formation since this counseling and has developed a method to ensure that he is always on time. As a military professional, he realizes the importance of personal responsibility to the unit and the Army.
