1. DUTY PERFORMANCE:
PFC L, your overall performance for the past month has been SATISFACTORY. This counseling is concerning your job performance since your initial counseling dated Nov 23, 2023. Overall, you have done an outstanding job. Please continue to take your duty and position as Dental Assistant in Oral Maxillofacial Surgery with an appropriate level of professionalism. During the month of November you were able to complete DMRSHI on your own, assist LTC Dang with pediatric patients, set up rooms for CPT Thomas and CPT Vazquez and familiarize yourself with the clinic and organization. Thank you for completing all tasks assigned to you, however make an effort to improve your work with little or no supervision.
2. APPEARANCE:
PFC L, over the past month you met the standards when it comes to your uniform and appearance and you complied with AR 670-1. Continue to maintain a good appearance for yourself and set the example for your peers to follow.
3. ACCOUNTABILITY AND RESPONSIBILITY:
PFC L, this month I have not had an issue with you reporting to work on time. Continue to do a good job and remember to always be at the right place at the right time in the right uniform. If you are ever running late, please notify me or SGT Gould.
4. SELF-DEVELOPMENT:
PFC L, you need to increase progress on your correspondence courses. I want you to complete at least 25 correspondence course hours every month. I will check your ERB monthly. I also encourage you to look into college and use your Tuition Assistance. Take advantage of the TA. Civilian education is also very important to progress in your Army career. If you have any questions, do not hesitate to ask me for guidance.
5. PROMOTION:
PFC L, you need to start looking at your next rank. If you want to be promoted, you need to start taking initiative and always be proactive in any situation that may come your way. In the future, keep in mind that you may want to prepare yourself for Soldier of the Month board. I will help you study and take the next step.
