DUTY PERFORMANCE: SPC C, your duty performance for the month of November was to standards. You do a great job of completing all tasks given to you with little or no supervision. You still need to complete your mandatory training for the new fiscal year.
APPEARANCE: SPC C, over the past month you met the standards when it comes to your duty uniform and appearance and you complied with AR 670-1. However, your coveralls for the motor pool are very dirty. You should try to clean them at least once a week so that they are not heavily stained. Continue to maintain a good appearance for yourself and set the example for your peers to follow.
PHYSICAL FITNESS: SPC C, you have always excelled in PRT. I would recommend that you take the initiative and conduct PRT for the group a few times a month. Continue to stay IAW AR 600-9.
ACCOUNTABILITY AND RESPONSIBILITY: SPC C, since October you have made a great improvement. I have not had any issues with you reporting to formations and work on time. Continue to do a good job and remember to always be at the right place at the right time in the right uniform. If you are ever running late, please notify myself or one of your peers so that we are aware of your situation.
SELF-DEVELOPMENT: SPC C, you need to make progress on increasing your correspondence courses. I know it's been a while since you last completed a course but I want you to complete at least 25 correspondence course hours every month. I will check your ERB monthly. I also encourage you to look into college and to use your Tuition Assistance. Take advantage of the TA. Civilian education is also very important for progression in your Army career. If you have any questions, do not hesitate to ask me for guidance.
SPC C, I have noticed that you want to do a lot of things such as attend the promotion board, get your USAREUR license, get a car, and learn the things that I do as an NCO. But you don't have a plan to reach your goals. SPC C, you can want everything in the world but how will you get if you don't have a plan of action? We will sit down together and make a plan so that you can reach your goals professionally and personally.
PROMOTION: SPC C, you need to start looking at your next rank. If you want to be promoted, you need to start taking initiative and always be proactive in any situation that may come your way. In the future, keep in mind that you may want to prepare yourself for the Soldier of the Month board. I have provided you with the MOI so it is up to you to study the material. I will help you with this process but you must take initiative if you want to attend the promotion board.
