MEMORANDUM FOR Commander, U.S. Army Human Resources Command, 200 Stovall Street, Alexandria, VA 22332
SUBJECT: Combat Action Badge Verification for SSG John Smith, U.S. Army
1. I verify that, while attached to Multi-National Force-Iraq (MNF-I) Headquarters, Camp Slayer, Baghdad, Iraq, SSG John Smith was engaged by small arms fire and mortar attack on 27 April 2004. SSG Smith was present at MNF-I headquarters in the course of his assigned duties when the building was attacked by anti-Iraqi forces. SSG Smith was on duty at the building entrance at the time of the attack and took an immediate defensive posture. Approximately 60 AK-47 rounds and eight mortar rounds impacted the front of the building and the area immediately in front of the compound. It is reasonable to state that SSG Smith could have been injured during the attack and performed his duties satisfactorily IAW established rules of engagement.
2. I was physically present at the ISG Headquarters, Camp Slayer, Baghdad, Iraq during the attack and am knowledgeable of SSG Smith's presence and response to the attack.
