Meritorious service while serving as Physical Therapy Technician at Naval Health Clinic Annapolis from February 2012 to March 2015. Sergeant Clarence Hollis' professionalism and unmatched technical ability were instrumental in the successful operation of the Bancroft and Hospital Point Physical Therapy Clinics. While at the Bancroft Hall Clinic, he provided more than 3,400 rehabilitative treatments to the active duty staff and the Midshipmen of the United States Naval Academy. In addition, he assisted in the opening of new Hospital Point Physical Therapy clinic and subsequently treated over 2,300 beneficiaries. Further, Sergeant Hollis demonstrated exceptional leadership while assigned as NCOIC of the Physical Therapy/Orthopedics Department. As NCOIC, he optimized patient scheduling by expanding clinic access, which resulted in a 20% increase of patient treatments at the Bancroft Hall Clinic.




MERITORIOUS SERVICE AS LEADING PETTY OFFICER, PHYSICAL THERAPY DEPARTMENT, NAVY MEDICINE READINESS AND TRAINING COMMAND, LEMOORE, CALIFORNIA FROM APRIL 2019 TO DECEMBER 2022. PETTY OFFICER BANTA PROFICIENTLY LED 10 STAFF MEMBERS ACROSS THREE HIGH VOLUME CLINICS, RENDERED CARE VALUED AT $1.9 MILLION, AND MAINTAINED A NINETY PERCENT RETURN TO FULL DUTY RATE. IN ADDITION, HE DEVELOPED A NINE WEEK CLINICAL COURSE, LEADING TO THE SUCCESSFUL GRADUATION OF ONE PHYSICAL THERAPY TECHNICIAN. BY HIS NOTEWORTHY ACCOMPLISHMENTS, PERSEVERANCE, AND LOYAL DEVOTION TO DUTY, PETTY OFFICER BANTA REFLECTED CREDIT UPON HIMSELF AND UPHELD THE HIGHEST TRADITIONS OF THE UNITED STATES NAVAL SERVICE.




SGT Jones averaged 143 wRVUs per month, exceeding the Tri-Service Annual Productivity Target for military Physical Therapy Technicians by over 30% each month. His diligent work resulted in an additional monthly revenue of $1.5K with a projected $12K additional revenue to the MTF for the mobilization period. SGT Jones ensured the highest quality of care and safety for over 900 patient visits, exemplifying LRMC's commitment as a High Reliability Organization.

SGT Jones proved to be an invaluable asset to the LRMC Physical Therapy (PT) team by consistently demonstrating safe, quality care and professionalism with every patient.
